Referrals //
For GP's
Private patients do not need a referral to be seen at Good Country Physiotherapy. However, if you feel that there is relevant information that will assist our physiotherapists to treat your patient's condition more effectively, we welcome your referral letter.
If your patient has a chronic disease and is being treated under a Medicare Chronic Disease Management Plan, we welcome these referrals also. Please note that we DO NOT bulk bill, unless there are extenuating circumstances which we are happy to discuss directly with your patient when they choose to book an appointment.
We welcome referrals for compensable patients injured at work or in a motor vehicle accident. Your usual referral letter is much appreciated to help us best manage these patients.
We welcome referrals for veterans under the Department of Veterans Affairs. Veterans are eligible for a referral for 12 months for up to 12 physiotherapy appointments. This referral should be provided on the appropriate paperwork from DVA.
For Case Managers of My Aged Care Package Clients
We welcome referrals for clients who have a My Aged Care package. Our rates for private patients are used and we will invoice their package manager directly. Where necessary, home visits can be arranged. We will keep you informed of your client's progress as we work together to improve their health outcomes.
Please use the form linked below to make your referral.
If your My Aged Care client is on the Commonwealth Home Support Program, we can only see these where a contract to provide these services is in place. If you are interested in working together to provide physiotherapy services for these clients, please contact our Director, Angela Willsmore at angela@goodcountryphysio.com.au or 0438 228 049 to discuss.
For Support Coordinators/Case Managers of NDIS Clients
We welcome referrals for clients who have funding via the NDIS. We are not currently registered by the NDIS Commission, so we are able to see those NDIS participants who are self-managed or plan-managed. Rates as set by the NDIA are used for consultations, travel and late cancellations. Our physiotherapists are happy to provide reports to assist in maintaining necessary funding. Please ensure these are requested with sufficient notice prior to any review meetings.
